In his second mental health assessment, Stephen answered yes on a form when asked if he had thoughts of suicide. He said he was thinking of harming himself "more than half the time." Instead of acting on Stephen Colley's answers, the social worker dealing with his assessment enrolled him in a sleep study to begin three weeks later.
